Data Verification Procedures
How IHEs May Check/Challenge/Change Data

A sample Program Completer / Tests Taken Matching Log is enclosed. This report will be made available to you via the Internet (August 28-Sept. 11, 2000 for 1998-99 data) so that you may check the program completer information recorded for your institution.

The first three columns provide name, partial SSN (the last 5 digits), and date of birth for each program completer identified for the programs within your institution (one line per program completer).

The Federal reporting requirement for HEA II deals only with initial certification and only with teaching titles. Therefore, if an individual was reported as being in a program for permanent (or professional) certification on the program completer lists you submitted, a special symbol ("~") appears in the next column and no further information is given. If the only certification codes for a program completer were for non-teaching titles, that individual will not be reported for Federal purposes. Up to three provisional certificate code columns may be filled with 4-digit codes indicating the teaching area(s) for which the individual was prepared.

The dates for highest exam scores recorded for the LAST and ATSW appear in the NYSTCE Exams columns. National Evaluation Systems, Inc., the company offering these tests, has listed dates rather than examination scores on this report since some test takers may have elected not to have their test scores reported to their college. You may verify these scores from the reports of tests scores sent to you after each test administration or from other files you may have on individual program completers.

Federal reporting requirements specify that institutions are only accountable for tests taken within five years prior to program completion. For 1998-99 program completers, only tests taken between 9/1/94 and 8/31/99 will be included in the report. On the verification report, an asterisk ("*") precedes any date outside that range. You will not be held accountable for scores from dates outside the range.

For individuals who used NTE Core Battery test scores to meet certification requirements, one or more check marks appear in the last three columns of the report (provided they took the test in the proscribed 5-year window and had the scores reported to the NYS Education Department).

If you think the data for your program completers are incorrect or incomplete, please follow the instructions that appear on the web with your verification report data. You must provide documentation of any error (i.e., copy of score report, documentation of change of name or SSN). Disputed information will be corroborated by the Office of Teaching at NYSED. Similarly, you will have to contact the Office of Teaching at [email protected] if program completers appear to have been omitted on your report, or if people have been included incorrectly.
